Below you will find the first round of paperwork needed to begin enrolling your account in the CMS sponsored clinical trial "POETRY".
Once the NDA, Site Qualification, and New Enrollment Forms have been filled out and signed, they need to be scanned and e-mailed back to admin@focus-hcs.com attn: Travis Wensjoe. If the account qualifies to participate in the POETRY study, a second round of paperwork will be e-mailed to you.
